From what I recall, this goes back to guidance dating to 1979 or earlier. At that time, the Soka Gakkai and Taisekiji Nichiren Shoshu were trying to resolve what would prove tp be irreconcilable differences. From 1979, until about 1991, Seikyo Times and/or Living Buddhism printed a series of articles by President Satoru Izumi, Vice President Takehisa Tsuji:

If I recall correctly, the original Tsuji method was to assign each of the 5 characters of Myo 妙 Ho 法 Ren 蓮 Ge 華 Kyo 経 to parts of the body. Like this:
Myo = head
Ho = throat
Ren = heart
Ge = stomach
Kyo = legs, arms, extremities
Nam was not assigned to a spot, it sort of surrounded and permeated the body. I would intone Nam as inclusive of the head, throat, heart, stomach, and extremities; then followed the schematic for myo ho ren ge kyo. As far as I know, Charles Atkins was the first to draw an inference from Tsuji's guidance to the chakras. I think his original schematic was something like this:

In 1991, my BP was 300/200; heart rate was 216. Soon after that, I was taking 30 mg Time-release Calcium Channel Blocker, 100 mg Cardio-Beta Blacker, & 30 mg ACE inhibitor. Even at those high doses, the control was poor. My BP was typically 170/110, hear rate 100.
In addition, I was on anti-verts and H1-blockers. Plus, various pain meds. The effects on my body were devastating. My hair fell out in patches. Weight loss from 190 to 130. I had kidney, bladder, & liver ailments. My equilibrium was very poor. Motion made nauseous. As of 2003, I also had aphasia, tachycadia, and an atrial fibrillation.
Today, I am taking 2.5 mg ACE inhibitor and 25 mg cardio-beta blocker. That is it. BP is 120/80; heart rate is 72. I take no pain meds. My balance is still poor, but I have an appetite and can walk without nausea. My heart rhythm is normal.
I started doing MVP and Zange {once again} in 2002, this time combined with Brahma-Vihara {4 palaces of Brahma: metta-karuna-mudita-uppekka; loving kindness, compassion, appreciative joy, equanimity} cultivations. In July of 2003, TSHTF. That is when I had the OOBE NDE.
For a time after that, I could not walk at all. I could barely compose a complete sentence on many days. But I was filled with an incredible bliss. Since then, improvement has been slow, not dramatic, but very steady.

"Visualise the mantra OM MANI PADME HUM and cause the mind to revolve from syllable to vsyllable thus:
OM -- heart
MA -- left shoulder
NI -- throat
PAD -- right shoulder
ME -- navel
HUM -- crown
Presently rearrange the syllables mentally and revolve the mind from one to another thus:
OM -- crown
MA -- throat
NI -- right shoulder
PAD -- navel
ME -- left shoulder
HUM -- heart
The more these revolving visualisations are repeated, the more marvellous the results."

Impressive. Funny thing about the articles you listed, specifically "The Key to Revitalization." When putting together the Living Buddhism articles for Modern Buddhist Healing, and again in some other quarters, I was challenged by the opinion that Mr. Tsuji was not being literal.
Ha!
I had archived every published speech, article, and English language word of Mr. Tsuji, and I proved that he meant exactly what he said - not what "they" supposed was some metaphorical take on daimoku and the body. Mr. Tsuji provided example after example of personal guidance he had given to a boy with a cancerous brain tumor, people with leukemia and other tumors, as well as all types of malady.
Mr. Tsuji instructed individuals to imagine the power of that daimoku sound to it's place on the body. His guidance was practical, literal, and direct. Moreover, it works!
It seems inconceivable that I was the first person to align Nam-myoho-renge-kyo to the chakras as Traditional Chinese Medicine was long ago mated with Ayurveda and the pranic energy systems and channels are completely implied. Therefore in the Ongi Kuden, when Nichiren likens myo to head, etc., He made the link.
